On November 7, 2023, I lost my father.
Not simply to illness, but as a direct result of care being unilaterally de-escalated—without disclosure, without consultation, and without consent.
My brother and I were his legally designated substitute decision-makers, and my father remained fully cognitively aware.
At no point were we informed that a decision to de-escalate care was being considered or implemented.
This was not an oversight—it was a clear violation of the standards set by the College of Physicians and Surgeons of Ontario. Their policy on decision-making for end-of-life care explicitly requires informed consent, transparency, and meaningful consultation with the patient or their substitute decision-makers.
None of those obligations were met.
The decision to withdraw and de-escalate care was made behind our backs and carried out without our knowledge—constituting a serious breach of professional and ethical duties owed to both the patient and his family.
When I later reviewed the records, I found documentation that did not reflect what we had been told, what we understood, or how events actually unfolded. This website exists to document those discrepancies, expose what happened, and push for accountability.
My father’s story is not just about one loss. It is about what can happen when medical power is abused behind closed doors, without transparency, without consent, and without regard for the families left to live with the consequences.
I am sharing this because no family should have to discover the truth only after it is too late.
Subsequent review of the medical record reveals inconsistencies, delayed charting, and omissions of material facts — particularly surrounding end-of-life discussions and the introduction of MAID-related conversations.
These omissions significantly alter the narrative presented by those responsible for his care.
The failure to document accurately, communicate transparently, and obtain informed consent is not a technicality — it is a fundamental breach of professional and ethical obligations.
This site exists to preserve the factual record, highlight these discrepancies, and ensure that these disgraceful and disgusting actions are not minimized or dismissed.
